The Northern State University athletic department has four full-time certified athletic trainers that work with coaches and athletes to prevent, diagnose and treat injuries. In accordance with NCAA and university policies, all incoming student-athletes MUST complete all required medical history and insurance forms before participation in any practice or competition. Those forms are available from the athletic training office.
Upon completion of those forms, athletes must sign off on the NSU Acknowledgment and Consent form and the HIPPA Privacy Consent form. Those forms are available for electronic signatures in the links below.
